Months of frustration ended
The challenge
Where we found them
A mental health platform had been in development for eleven months with the previous team and was still not usable. The architecture decisions could not support the compliance, multi-tenancy, or AI-driven features the business required. Iterative patching had reached the end of its useful life — the only safe path was a ground-up rebuild on a stack designed for healthcare-grade reliability.
Our approach
How the fleet ran it
We treated this as a new-development engagement on the pavel-* reference stack: Next.js, Prisma, PostgreSQL, encrypted patient-provider messaging, OpenAI-backed assessments, role-based access control with granular permissions, and a multi-tenant data model from day one. Every architectural decision went through a HITL review against the compliance checklist before code was written.

The outcome
What landed
A complete platform shipped on the modern stack with multi-tenant architecture, end-to-end encryption on patient communication, AI-powered recommendations, automated assessment scoring, and analytics dashboards. The client moved from eleven months of frustration to a production-ready system in a single engagement window.
How it is run
A person approves anything that carries risk
Every engagement runs the same four stages — map, build, run in parallel, then operate — and you hold the gate at each one. Agents do the repeatable part; anything that contacts your customer, moves money or records a regulatory conclusion is approved by a person first, and the approval is recorded against the case.
